Who was the longest distance from Earth?

"Houston, we've had a problem."

These words were first uttered by John L. Swigert aboard Apollo 13 on April 13 1970 after an oxygen tank explosion.

Actually these famous words were relayed to ground control on several occasions. James A. Lovell said them as well.

When Apollo 13 rounded the far side on the Moon on April 15 1970, 158 miles from the surface, James A. Lovell, John L. Swigert and Fred W. Haise set a new record for the longest distance from Earth being 400,171 km or 248,655 miles away.
